1. We would like a short paragraph that states:
- Who you are.
- How much you’re asking for.
- What you intend to do with the grant.
- How may we contact you?
- Who to make the check out to?
(We would prefer you to attend the event at the end of the year so we may present you with the check in person)

2. If you’re not a non-profit, we will need the name of the non-profit that you use as your fiscal agent/sponsor and a contact that we may call for any questions.
3. Please include a copy of your Internal Revenue Service Letter of Determination of 501(c)3 Non-Profit Status.
4. Please show the reason you are requesting a grant, i.e.: startup costs, equipment not covered by normal operating budget, program incentives, direct service cost and scholarships.

Please keep in mind we’ll only approve your request if we feel that you can make an impact in our community. Also we can not approve any grant request that involve capital campaigns, funding for salaries, general operating expenses, individuals projects, or travel expenses.
